Forms appended to the Rules of Practice and Practice Directions of the Environmental Review Tribunal are available electronically.

Adobe PDF Forms can be completed on-line, printed and filed with the Tribunal. If you have Adobe Acrobat you can save the form and send it as an e-mail attachment to ERTTribunalSecretary@ontario.ca (only Forms 3 & 4 are accepted electronically).

Available Forms:

	Appeals/Applications Form -- Filing appeals or applications electronically. The Environmental Review Tribunal, Niagara Escarpment Hearing Office and Office of Consolidated Hearings accepts the filing of certain appeals or applications electronically.
	Form 1 -- Notice of Constitutional Question. To be used when a Party has raised a constitutional issue in a Hearing. It must be served upon both the Attorney General of Ontario and the Attorney General of Canada, as well as all other Parties to the Hearing. A signed original Statement of Service (Form 2) or an affidavit of service, together with the Notice of Constitutional Question, must be filed with the Tribunal.
	Form 2 -- Statement of Service. To be used as an equivalent to an affidavit of service to prove that a Party has served certain documents upon another Party or Parties in a Hearing before the Tribunal. This form must be printed and a signed original filed with the Tribunal.
	Form 3 -- Summons to witness (Oral Hearing). To be used when a Party wishes to request that the Tribunal issue a summons to an individual to appear before the Tribunal at an oral Hearing. This form can be completed on-line and either submitted electronically, by facsimile or by regular mail.
	Form 4 -- Summons to witness (Electronic Hearing). To be used when a Party wishes to request that the Tribunal issue a summons to an individual to participate in a Hearing or motion before the Tribunal to be held by telephone conference call. This form can be completed on-line and either submitted electronically, by facsimile or by regular mail.
